When planning for your appointment at OrthoSouth, if you choose not to complete, print out and bring with you OrthoSouth's patient registration forms (NEW PATIENTS ONLY), please be sure to arrive 15 to 20 minutes early in order to complete the necessary paperwork.
It is important ALL patients bring to their scheduled appointment any x-rays, MRI exams, doctors notes, operative notes or exam reports. These are necessary resources that allow your OrthoSouth surgeon to offer a conclusive diagnosis of your injury at the time of service.
Patients also are reminded to bring ALL previous medical charts/records, a photo ID, and an updated insurance card. If you are not in possession of your medical records, please contact your physician's office and request your records be sent to our office. We may need your written permission in order for the records to be transferred. If you are not able to have your medical records sent in time for your appointment, or are unable to bring them with you, we may need to reschedule your appointment.
If you are unable to keep a scheduled appointment, please call OrthoSouth's main office number as soon as possible to reschedule. If you have an urgent medical problem and need to request a same day appointment, please call our office as soon as possible and we will make every attempt to accommodate you.
